  • Veteran Status: Unknown, Yes, No, Not Provided Protected Veteran: If Veteran Status is Yes, then this option appears.

  • Driver License Eligibility: Not Applicable, Yes, No. Monitor, Limited

  • Ethnicity

  • Preferred Pronouns

  • Exempt from COVID-19 Vaccine
