Provider Listing Report

Description: This report displays a list of all provider accounts assigned to the selected access group. It includes general information about each account.

Use: Use this report to review a list of all provider accounts.

Report Output: Each column of the report displays:

Columns	Description
Provider Medicaid ID	the state assigned identifier for the provider
Account Number	the provider's Sandata EVV account number
Account Name	the account name
Account Address	the account's address
Date Created	the date the provider account was created by Sandata
