Referral Source Detail Report

Description: This report displays a list of clients who have been referred to the agency, sorted under their referrer. This report displays subtotals that can be used for statistical analysis.

Use: This report can be used by the agency to view statistics, e.g. retention rate of referrals, frequency of referral and common referral sources.

Limitations: N/A

This report can be filtered by:

Field	Description
Company	The selected company.
Location	The selected location.
Admit Type	The code for the client's admission.
Client Status	The selected client status.
Referral Source	The selected referral source.
Referral Type	The selected referral type.
Marketer	The selected marketer.
Date From	The first date of a selected range of dates.
Date To	The last date of a selected range of dates.

This report can be sorted by:

Field	Description
Client Name	Sorts the report by client name in ascending or descending order. Blank if unknown.
Source	Sorts the report by referral source in ascending or descending order.
Referral Date	Sorts the report by the date of the referral in ascending or descending order.
Client Status	Sorts the report by client status in ascending or descending order.
Contact	Sorts the report by the contact's name in ascending or descending order.

Each column of the report displays:

Field	Description
Name	The client's name (last, first).
Chart ID	The client's Chart ID number.
Services	The type of care provided.
Physician	The client's primary physician.
Contact	The client's primary contact.
Marketer	The client's marketer.
Referral Date	The date of the client's referral.
SOC	The client's start of care date.
Status	The client's status.
Total	The total number of clients per referral source.
Total Referrals	The total number of clients on the report.
