Client Medication Profile Report

Description: This report lists client health and medication information. Each active medication is listed below.

Use: This report can be used by the agency to provide a medication sheet to either the client or a designated caregiver.

Limitations: N/A

This report can be filtered by:

Field	Description
Company	The selected company.
Location	The selected location.
Admit Type	The code for the client's admission.
Team	The selected team.
Client	Client by name (last name, first name) or Sandata Client ID. When a partial name is entered the report displays results that begin with the entered characters.
Client Status	The selected client status.
Date	The selected date.

Each column of the report displays:

Field	Description
Allergies	The client's allergies (if applicable).
DME	Durable Medical Equipment the client requires.
Safety	Safety information for the client.
Nutrition	The client's nutrition/diet.
Client	The client's name (last, first).
Address	The client's primary address.
Phone	The client's phone number.
Primary Diagnosis	The client's primary diagnosis.
Physician	The client's primary physician.
Pharmacy	The client's pharmacy.
Start	The start date of the medication.
End	The end date of the medication.
Name	The name of the medication.
Strength	The strength of the medication in units.
Dose	The dosage of the medication.
Freq/Rout	The frequency and route of administration of the medication.
Classification	The classification of the medication.
Med Comments	Comments for the medication.
