Client Discharge by Reason Report

Description: This report displays discharged clients sorted by reason for discharge.

Use: This report creates a grouped list display that can be used to evaluate reasons for client discharges.

Limitations: N/A

This report can be filtered by:

Field	Description
Company	The selected company.
Location	The selected location.
Team	The selected team.
Admit Type	The selected admission type.
Reason	The selected reason code.
Date From	The selected starting date for a date range.
Date To	The selected ending date for a date range.

This report can be sorted by:

Field	Description
Client Name	Sorts the report by client name in ascending or descending order. Blank if unknown.
End of Care Date	Sorts the report by the client’s end of care date.

Each column of the report displays:

Field	Description
(Discharge Reason)	The client’s discharge reason.
(Client) Name	The client’s name (last, first).
Chart ID	The client’s Chart ID.
Physician	The client’s primary physician.
SOC (Start of Care)	The client’s Start of Care date.
EOC (End of Care)	The client’s End of Care date.
Total (Discharge per Reason)	The amount of discharges for each reason.
(Discharge) Report Total	The total amount of discharges.
