Client Birthdays Report

Description: This report displays a list of birthdays for clients active during a selected date range. The report includes the client’s name and contact information as well as their start of care date and birthday.

Use: This report can be used to reference a client’s date of birth for event planning or other situations where a date of birth may be needed.

Limitations: N/A

This report can be filtered by:

Field	Description
Company	Use this dropdown to select the company.
Location	Use this dropdown to select the location.
Admit Type	Use this dropdown to select the admission type.
Team	Use this dropdown to select the team.
Client Status	Use this dropdown to select the client’s status.
Staff Manager	Use this dropdown to select the staff manager.
Coordinator	Use this dropdown to select the coordinator.
Date From	Use this field to search for clients beginning on this date.
Date To	Use this field to search for clients ending on this date.

Each column of the report displays:

Field	Description
Name	Displays the client’s name.
Home Phone	Displays the client’s home phone number.
Chart ID	Displays the client’s customer number.
Physician	Displays the client’s physician’s name.
Manager	Displays the manager associated with the client.
SOC	Displays the client’s start of care date.
Birthday	Displays the client’s birthday.
