Authorization Requests by Referral Source Report

Description: This report displays a list of client authorization requests sorted by referral source, along with information needed for authorizations.

Use: This report can be used by the agency to view client information. This can be used in requesting additional authorizations.

Limitations: N/A

This report can be filtered by:

Field	Description
Company	The selected company.
Location	The selected company.
Admit Type	The code for the client's admission.
Payor	The selected payor.
Team	The selected team.
Referral Source	The selected referral source. Varies by agency.
Client Status	The client's status: Pending, active, on hold, discharged, canceled.
Date From	The first date of a selected range of dates.
Date To	The last date of a selected range of dates.

Each column of the report displays:

Field	Description
Client	The client's name. Blank if unknown.
Cust No.	The ID number assigned by the payer.
Assessment Due Date	The date of the next required health assessment for the client.
Other ID	Any additional ID number the client may have.
Zip Code	The client's zip code.
Admission Type	The code for the client's admission.
