Active Client Roster Report

Description: This report lists all active clients for the provider agency sorted by coordinator. Clients with an unknown coordinator will display at the top.

Use: This can be used by the agency as an overview of all active patients with key data points for each admission.

Limitations: N/A

This report can be filtered by:

Field	Description
Company	The selected company.
Location	The selected location.
Team	The selected team.
Admit Types	The code for the client's admission.
Is Client Clinically Integrated	The selected clinical integration status (yes/no).
Coordinator	The selected coordinator.
Manager	The selected manager.
Marketer	The selected marketer.
Payors	The selected payor.
Multiple Client Status	The selected client status.
Date	The selected date.

Each column of the report displays:

Field	Description
Client Name	The client's name (last, first).
Chart ID	The client's Chart ID number.
SOC	The client's start of care date.
Current Cert Period	The amount of time the client is eligible for the program; the certification period.
Diagnosis	The diagnosis code and brief description.
Services Provided	The type of care provided.
Clinically Integrated	The client's clinical integration status (yes/no).
Clinical Integration Effective Date	The date of the client's clinical integration. Blank if no.
