Sandata Agency Management 08-CAHPS Survey Screen

The Consumer Assessment of Healthcare Providers and Systems (CAHPS) Survey screen provides information associated with completed client surveys for exporting and use in an agency’s response to the CAHPS Survey.

To access the CAHPS Screen, go to Main Menu > Client > Exports > CAHPS Survey.

The search filters can be used to locate completed surveys using the search terms. If no search terms are selected, all results are displayed.

Field	Description
Company	Use this dropdown to filter by company.
Month	Use this dropdown to filter by month.
Year	Use this dropdown to filter by year.
CMS Only	Select this checkbox to filter by results matching CMS.

The results list displays all results matching the selected terms in the search filters.

Field	Description
Client	Displays the client’s name.
ChartID	Displays the client’s chart identification number.
Type	Displays the type of OASIS assessment completed.
Assess	Displays the date of the assessment.
HIPPS	Displays the HIPPS code.
